About The Position
Position Description
Are you passionate about making a meaningful impact on the lives of the geriatric population? We are seeking a compassionate and experienced Nurse Practitioner to provide comprehensive primary care services to residents in assisted living facilities in Fulton County, NY. The ideal candidate will have a background in geriatrics and / or long-term care and a dedication to delivering high-quality, patient-centered care.
In this rewarding role, you will perform comprehensive health assessments, develop personalized care plans in partnership with the interdisciplinary team, manage chronic and acute conditions, order and interpret diagnostic tests, and provide patient and family education. Enjoy a work environment with no nights or weekends and flexible full-time or part-time schedules.

Requirements
- Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program
- Unrestricted New York State Nurse Practitioner license
- Valid DEA permit to prescribe medications commonly used in practice
- Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid
- Minimum of 3 years clinical experience, preferably in long-term care or geriatrics
- Experience with eClinicalWorks (eCW) electronic health records preferred
