Swing Bed Activity Aid
Swing Bed Activity Aid plays a vital role in enhancing the quality of life for patients in the swing bed program. Planning and implementing a variety of therapeutic and recreational activities designed to improve the physical, mental, and emotional well-being of patients transitioning from acute care to long-term recovery. The Activity Aid works closely with the Swing Bed Program Coordinator, nursing staff, Physical / Occupational Therapy, and other healthcare professionals to ensure that patients receive comprehensive, holistic care. Essential functions include greeting and assisting patients or visitors who present to the nurses' station, assessing patients' activity preferences, hobbies, abilities, and limitations upon admission and throughout their stay and documenting in electronic health record, developing and facilitating daily recreational and therapeutic activities tailored to individual patient needs and interests, reporting observations and any changes in patient condition to the healthcare team, assisting with participation with PT / OT exercise, ambulation, and passive Range of Motion, restorative exercise room as prescribed by therapist, coordinating group activities such as arts and crafts, games, exercise sessions, and social events, participating in interdisciplinary team meetings to discuss patient progress and activity outcomes, and ensuring a clean, safe, and organized environment for activities. Provides or assists assigned patients with their daily care activities according to established procedures and with consideration for the patient's choices, privacy, dignity, and independence. This includes assisting patients / residents with bathing, dressing, undressing, assuring clothing is clean and properly marked, serving and collecting food trays, encouraging and / or assisting with feeding, answering call-lights, transporting patients or assisting them in ambulating, and assisting with admission and orientation of patients / residents to the facility environment. Assist with stocking inpatient rooms. Keep rooms tidy and clear from clutter daily. Schedule follow up appointments upon discharge and communicate to patient date and time of appointments. May assist in the giving of instructions to patients regarding follow up appointments. Maintain patient confidentiality. Collaborate with other departments to schedule patient tests / procedures. Other duties as assigned. Knowledge, education, and experience required include a high school diploma or GED, medical assistant or certified nursing assistant certification preferred, medical terminology desired, BLS for healthcare provider, and computer competency.
