Description :
This position functions within the Behavioral Health Division to clinically manage a panel of patients.
The focus is on skill-building, guidance, psychoeducation, supportive counseling, coaching, and strength-based encouragement for individuals experiencing behavioral health disorders or serious mental illness.
The position also assists patients with medication refills, referrals, triage, and patient education.
Key essential functions include meeting with assigned patients to monitor medication side effects, compliance, and progress toward treatment goals.
Accurate and timely documentation of services must be maintained in accordance with policy.
Coaching, guidance, and support are provided using a strength-based approach toward recovery as described in the patient treatment plan.
Individual and family education, training, and support with self-help skills that promote wellness and stability are integral to the role.
Facilitation of medical care for patients in Behavioral Health / Psychiatry Service is required.
The position includes facilitating medication refills between patients, physicians, pharmacies, and insurance providers.
Nurse case management is necessary to identify and obtain appropriate medical and therapeutic information in preparation for prescriber visits.
Patient education is provided, and referrals to physicians are made as appropriate.
Medication injections and lab collections may also be conducted.
Coordination of care plans with primary care and specialty services is essential.
The development and management of individualized treatment plans for the assigned caseload are expected.
Culturally sensitive, systematic mental health nursing care that reflects contemporary practice and the patient’s treatment plan must be provided.
Utilizing brief patient contacts to reinforce plans of care is necessary.
Identification of situations where additional services would benefit patients is required, along with communication with clinicians.
Researching patient education materials and ensuring clinic equipment is maintained are part of the role.
Other duties may be assigned as needed.
