Full time Bilingual LVN Service / Care Coordinator openings in the Rio Grande City, Texas area with a major healthcare company! Starting ASAP!! Apply now to Chris Meyer with A-Line!

Position is hybrid with 3 days of travel and 2 days of remote work per week, travel will be limited to the area (Will be reimbursed for travel)

Service / Care Coordinator PAY RATE : $25.00-$28.00 per hour

Service / Care Coordinator HOURS : M-F 8am-5pm Central Time

Service / Care Coordinator Job Description :

  • Complete Face to Face, Zoom, or Telehealth assessments, up to 4 assessment per day x3 days per week
  • Complete required forms for all assessments completed within required time frames, Documentation days x2
  • Work assigned tasks / assignments to address member needs / requests
  • Work other possible assignments such as Complaints & Escalations
  • Possible high priority assignments that will require urgent attention
  • Update member record as needed

Describe the performance expectations / metrics for this individual and their team :

  • Complete documentation within 3-day turnaround time
  • Maintain a 90% or high in Audit scores

    • Service / Care Coordinator Required Skills :

  • Must be Bilingual - able to fluently read and speak English and Spanish
  • Ability to travel 3 days per week (will be reimbursed for mileage)
  • Active LVN / LPN / LMSW License required.
  • 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions
  • Strong documentation skills
  • Excellent computer skills
  • High School Diploma or GED

    • Service / Care Coordinator Preferred Skills :

  • Service Coordination, Case Management or Nursing experience preferred.
  • Background in Managed Care
  • Familiarity and experience working with Medicare or Medicaid plans
  • Degree preferred
