About the Role
The NP will provide comprehensive health assessments, diagnosis, treatment, and patient education in collaboration with the care team, ensuring continuity of care across virtual and onsite settings. This role requires clinical expertise, adaptability, and strong communication skills to deliver safe, evidence-based care in multiple environments.
Responsibilities
Clinical Care (Onsite & Remote)
- Perform patient evaluations, physical exams, and diagnostic assessments.
- Diagnose and manage acute and chronic conditions within scope of practice.
- Prescribe medications, order diagnostic tests, and interpret results.
- Provide follow-up care and monitor treatment outcomes both virtually and in person.
- Conduct wellness visits, preventive screenings, and health risk assessments.
Patient Education & Advocacy
Educate patients and families on disease prevention, health promotion, and treatment plans.Support care transitions, including discharge planning and referrals.Serve as a patient advocate, ensuring individualized, culturally competent care.Collaboration & Teamwork
Partner with physicians, RNs, case managers, and interdisciplinary care teams to coordinate care.Communicate effectively across virtual platforms and in-person team meetings.Participate in care planning, quality improvement initiatives, and case conferences.Compliance & Professional Standards
Maintain accurate documentation in the electronic health record (EHR) for both onsite and telehealth encounters.Adhere to HIPAA, state / federal regulations, and organizational protocols.Qualifications
Current, unrestricted Nurse Practitioner license in (State(s)) (multistate / compact or additional licenses preferred).Graduate of an accredited Nurse Practitioner program (MSN or DNP required).Board certification as an NP (FNP, AGNP, ACNP, or specialty area).Minimum of 2–3 years of NP clinical experience (telehealth experience strongly preferred).Current BLS / ACLS certification.Strong diagnostic, clinical, and telecommunication skills.Proficiency with EHRs, telehealth platforms, and digital health tools.Preferred Skills
Experience in either primary care, urgent care, cardiology, or oncology.Familiarity with remote patient monitoring tools and digital health integration.Bilingual (e.g., English / Spanish) a plus.Work Environment & Schedule
Combination of onsite clinic / hospital hours and remote to your discretion. Flexible scheduling, including occasional evenings or weekends based on patient needs.
