Case Manager
Beyond Home Health Care is seeking a talented and dedicated Case Manager to join the team. This position is located in Toms River, NJ with flexibility for remote work on Friday.

Position Summary
This position plays a vital role in coordinating care and ensuring quality services while also leading recruitment efforts for Direct Support Professionals (DSPs) and therapeutic service providers to meet individual needs.
Responsibilities of the Case Manager :
- Create and post jobs as needed and ensure all ads are accurate and up to date.
- Screen resumes and applications to identify qualified candidates.
- Collaborate with interdisciplinary teams including support coordinators, DSPs, therapists, and other providers.
- Conduct initial phone / video interviews to assess candidates’ skills, experience, and cultural fit.
- Maintain an active roster of qualified, available DSPs and therapy providers for timely client placements.
- Oversee the intake process for new participants and manage active cases.
- Supervise Direct Support Professionals (DSPs) actively working with clients.
- Ensure quality care and adherence to schedules by DSPs.
- Monitor authorizations and ensure continuity of services for participants.
Requirements of the Case Manager :
Experience working with Applicant Tracing Systems, specifically Apploi and Indeed.Excellent organizational, communication, and problem-solving skills.Ability to work independently and as part of a multidisciplinary team.Previous experience working in recruiting, preferred.
