About the Role
Are you passionate about helping older adults and individuals with disabilities live safely and independently in their homes? We're seeking a Certified Aging in Place Specialist (CAPS) Case Manager to join our team and make a meaningful impact across Central Florida.
In this role, you'll conduct in-home assessments to evaluate safety, accessibility, and functional needs. You'll recommend and follow up on home modifications—like grab bars, wheelchair ramps, and other adaptive solutions—to improve quality of life for our clients, most of whom are 62+ or disabled adults.

Qualifications
Occupational Therapist (OT) or Occupational Therapy Assistant (OTA) license.
CAPS certification (we'll pay for your 3-day online training if you don't have it).
