  • ID License or IMLC : Required
  • DEA / CSR : Required
  • Comfortable with Regional Anesthesia : Required
  • Must be Comfortable Performing Own Cases or Working in Supervising / Care Team Model : Required
  • Proof of COVID-19 Vaccine : Required (Verbal confirmation at time of name clear)

    • Practice Details :

  • Seeking a full-time locum Anesthesiologist starting May 2024
  • Work at both Nampa and Boise locations
  • Comfortable with Regional Anesthesia
  • No Cardiac / Complex Peds / Transplants
  • OB Coverage : Required for Nampa, Not Required for Boise

    • Credentialing Requirements :

    Ensure the following items are readily available for the provider at the time of acceptance to expedite the credentialing process :

  • MD Diploma
  • Internship / Residency / Fellowship Certificates
  • All Medical Licenses
  • DEA
  • Board Certification
  • Life Support Cards (BLS)
  • Current and Previous Certificates of Insurance
  • Drivers License
  • Professional Color Photo / Headshot
  • CMEs for the Past 2 Years
  • Immunization Records (TB, Flu, HepB, Tdap, Varicella, COVID)
  • References
