Job Summary and Qualifications

1. Perform appropriate assessments to include psychological, physical, environmental & functional needs.

2. Provide patient & family education specific to the patient’s disease process.

3. Serve as a resource for education for patient, family, staff, hospital & community.

4. Communicate with all members of the transplant team to include hospital staff & referring physician / staff

5. Participate in the collection / entry of data in the patient medical record as required by all regulatory agencies

6. Document assessments, interventions & follow-up accurately & timely

7. Coordinate care through the phases of transplantation with the transplant team.

What qualifications you will need :

Mandatory : Graduate of an accredited registered nursing program.
