Join Our Team as a Home Care RN Case Manager!
As a Home Care RN Case Manager, you will have the opportunity to work independently in the field, providing personalized care to patients in the comfort of their homes. This role involves comprehensive head-to-toe assessments, care coordination, disease and medication management, skilled wound care, catheter management, ostomy education, infection prevention, and promoting home safety.
Compensation and Benefits :
- Annual Salary Range : $89,419 - $106,620
- Mileage Reimbursement / Company Car Program

Work Schedule : Full-time | Day Shift | Monday - Friday | Holiday rotation (work about two holidays per year) | Weekend rotation may apply based on staffing needs.
Work Territory : This position will involve visiting patients in their homes throughout the South East section of the Twin Cities Metro area. The office is located in Roseville, MN.

Graduated from an accredited nursing program and hold a current RN license in the State of MN.Ability to travel within the designated geographic area, requiring reliable transportation, valid driver's license, and proof of auto insurance.Current CPR Certification.Preferred : Two years of nursing experience and one year of case management experience.Preferred : Experience in community health and a diverse range of Medical-Surgical care.Your Responsibilities :
Conduct thorough assessments and procedures using your nursing training and skills rooted in psychological, biological, physical, and social sciences.Evaluate in-home care quality and review clinical notes during patient visits.Participate in ongoing interdisciplinary patient assessments.Deliver services as per physician’s orders detailed in the care plan.Provide counseling and education to patients and families to meet healthcare needs.Document clinical notes accurately and timely, focusing on patient progress.Communicate effectively with the interdisciplinary healthcare team to ensure coordinated patient care and plan for discharges.If you're ready to make a difference in patients' lives and join a fantastic team, we encourage you to apply!
