What You'll Do as an RN, Case Manager
As part of the Redirect Health team, you'll play a vital role in care management by :
Population Health Management : Proactively monitor and manage the 1% high-risk population list to prevent avoidable hospitalizations and ER visits.Care Coordination : Develop, implement, and evaluate individualized care plans for members with complex medical and social needs.Member Advocacy : Serve as a trusted nurse advocate, helping members navigate healthcare resources and ensuring timely follow-up care.Collaboration : Work closely with providers, care teams, and community partners to deliver integrated, member-centered care.Outreach & Engagement : Communicate with members and their families via phone to provide education and support.Data-Driven Insights : Track outcomes and trends, identifying opportunities for early interventions and improved care delivery.Clerical & Administrative Duties : Accurately document all member interactions, maintain organized records, complete required forms, and support data entry and reporting tasks to ensure seamless operations.Team Support : Work with the support of a dedicated Medical Assistant and Medical Director, ensuring clinical collaboration and shared accountability in patient care.What We're Looking For
Active RN license in Arizona (or compact state licensure), required.ER and Hospital experience required, with the ability to triage and manage recently discharged hospital patients.2+ years of clinical nursing experience, with case management or population health experience strongly preferred.Strong critical thinking, communication, and care coordination skills.Ability to work collaboratively across teams and with diverse patient populations.Friendly, empathetic, and resourceful problem-solver dedicated to improving member outcomes.Bachelor of Science in Nursing (BSN), preferred.What You'll Earn
