Responsibilities : The role of the Medical Care Counselor is to support and provide health-related advocacy for individuals residing in our group homes for developmentally disabled adults.

Key activities include :

• Providing continuous protective oversight to the individual
• Scheduling transporting and accompanying individuals to appointments with providers; facilitating communication with providers re : individuals health changes
• Maintaining all health-related documentation including lab work results consultation reports and updated medication prescriptions
• Communicating daily with nursing and residential management re : individuals medical status and treatment plans
• Managing medication inventory including prescriptions over-the-counter medications nutritional supplements and first aid supplies
• Providing companionship visits to individuals that are hospitalized as required

Qualifications :

Work Schedule : 40 hrs. / wk. (6a-2p Mon-Fri)

Salary : $23 / hr.

This rate of pay serves as a good faith estimate. Actual pay is based on a number of factors which may include a candidates qualifications skills competencies experience and work location. The salary range or rate listed does not include any differential pay or other forms of compensation such as the value of benefits which may be applicable to this position.
