Job Summary and Qualifications

As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Their main focus is advocating for patients, families, and caregivers while embodying the organization's vision, mission, and values. The goal is to give an unmatched patient experience and attain optimal clinical outcomes.

What will you do in this role :

patients / families / caregivers

about a patient’s medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete knowledge.

/ families / caregivers

receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.

Supervisors / Nursing

Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
