Your Role

As a Case Manager, you will provide in-home care to former Nuclear Weapons Workers facing chronic and terminal illnesses due to their work environment. Your responsibilities will include :

• Conducting weekly nursing visits lasting 1-2 hours.
• Performing physical assessments, including vital checks.
• Ensuring home safety with medical equipment and supplies.
• Filling weekly medication planners.
• Overseeing skilled and non-skilled caregivers in the client's home.
