Responsibilities
Provide high-quality patient care in a skilled nursing facility.
Participate in ongoing professional development and training programs.
Engage in continuous learning to enhance skills and knowledge.
Collaborate with interdisciplinary teams to develop patient care plans.
Achieve performance targets and contribute to team success.
Monitor and document patient progress and outcomes regularly.
Maintain high visibility of work activities and results.
Communicate effectively with patients and their families.
Utilize pipelines to assess potential success in patient care.
Ensure compliance with healthcare regulations and facility policies.
Participate in quality improvement initiatives within the facility.
Provide mentorship and support to junior staff as needed.
Experience / Qualifications
Valid occupational therapy license in the relevant state.
Eagerness to learn and adapt in a fast-paced environment.
Experience in skilled nursing facilities or similar healthcare settings.
Commitment to ongoing professional development and training.
Ability to set and achieve performance targets independently.
Strong communication skills for effective patient and family interactions.
Proven ability to collaborate with interdisciplinary healthcare teams.
Familiarity with healthcare regulations and compliance standards.
Ability to monitor and document patient progress accurately.
Experience in quality improvement initiatives within healthcare settings.
