Job Summary
Functions as an integral member of an oncology pharmacy team to provide care for patients in collaboration with physicians, nurses, other providers and support personnel in an ambulatory clinic setting. Performs comprehensive patient education for in-clinic chemotherapy, outpatient chemotherapy and all pharmacologic supportive therapy. Interprets physician chemotherapy orders, conducts pretreatment patient assessment in conjunction with nurse partners, processes and dispenses medication orders in compliance with Colorado State Board of Pharmacy and federal regulations. Serves as a drug information resource for patients and providers. Maintains standardized documentation in the patient electronic medical record detailing therapeutic actions or recommendations. Prepares and dispenses injectable chemotherapy in a USP
compliant setting, coordinates outpatient chemotherapy and all supportive medication needs with the main outpatient pharmacies and responds quickly to meet member / patient needs.
Essential Responsibilities
This position, knows and complies with all Kaiser Permanente quality, safety, and emergency policies and procedures. Demonstrates quality and effectiveness in work habits and clinical practice in every interaction with patients, colleagues, providers, and leadership. Ensures patient safety in the preparation and provisioning of care related to but not limited to medications, procedures, infection prevention, fall prevention, including consistent use of two patient identifiers and procedural time outs. Reports safety hazards, accidents and incidents, and unsafe working conditions promptly.
Prepares, packages, stores and distributes intravenous and oral chemotherapy medications for oncology patients within established procedures and legal requirements.
Reviews physician orders and monitors patient drug therapy and laboratory test results. Provides drug information when needed.
Serves as the primary role in patient education for the oncology clinics.
Orders and maintains drug inventory and equipment necessary for the delivery of oncology chemotherapy.
Performs biological and hazardous waste disposal.
Maintains records of drugs and controlled substances as required by federal and state law.
Keeps abreast of current drug information as it relates uses, dosages and side effects.
Performs other pharmacist duties as required.
Basic Qualifications
Experience
Minimum one (1) year of experience required as a registered pharmacist which includes counseling patients regarding appropriate use of medications, educating providers regarding medication use and selection, and directing the work of pharmacy support staff.
Minimum one (1) year of work experience required in the areas of IV preparation and aseptic technique.
