Responsibilities :

Create and implement personalized treatment plans for home-based pediatric and adult therapy patients

Conduct initial and ongoing comprehensive assessments of the patient's needs

Collaborate with interdisciplinary teams to ensure comprehensive care and support for patients and families

Maintain accurate and timely documentation of patient progress and treatment plans to follow Colorado Home Health guidelines

Demonstrate a strong foundational knowledge of speech-language pathology and the ability to effectively problem-solve in diverse situations

Track hours and maintain ASHA compliance for completion of CFY year

Requirements :

Must hold a Master's degree in Speech Therapy from an accredited program

Licensed to practice Speech Therapy in the state of Colorado

CPR certification

Maternity Leave Coverage needed starting January 2026)

Pay Structure :

$55 per regular visit! (30-60 minute visits at your clinical discretion)

Start of Care Evaluations- $75 / visit

Recertifications / Re-Evaluations (Every 60 days)- $60 / visit
