Responsibilities :
Create and implement personalized treatment plans for home-based pediatric and adult therapy patientsConduct initial and ongoing comprehensive assessments of the patient's needsCollaborate with interdisciplinary teams to ensure comprehensive care and support for patients and familiesMaintain accurate and timely documentation of patient progress and treatment plans to follow Colorado Home Health guidelinesDemonstrate a strong foundational knowledge of speech-language pathology and the ability to effectively problem-solve in diverse situationsTrack hours and maintain ASHA compliance for completion of CFY yearRequirements :
Must hold a Master's degree in Speech Therapy from an accredited programLicensed to practice Speech Therapy in the state of ColoradoCPR certificationLocations of Need :
