We're looking for someone to join our team as a Lead Care Coordinator-ECM who :

The Lead Care Coordinator will oversee the Enhanced Care Management (ECM) schedule for the Care Coordinators (CC) and will provide guidance to field staff regarding routine patient assistance, technical and supportive functions under the direction of the Supervisor. The Lead Care Coordinator will conduct routine audits with field staff including joint visits and documentation overview. Acts as a preceptor / mentor to other CCs and is a subject matter expert in CC functions. This position will demonstrate strong leadership and communication skills. The Lead Care Coordinator is expected to participate in continuous improvements in quality and the patient experience. This position will model, train, and promote high functioning support staff from all experience levels.

Essential Functions :

Assist the field staff in achieving excellence and promoting a positive work environment.

Review charts, and encounters for completeness and assess follow-up as needed to close care gaps and / or Community resources.

Demonstrate and model excellent customer service skills and assist the Program Supervisor and Field Care Coordinators in providing timely resolution to patient concerns as needed.
