Position Overview

The Hospitalist Nurse Practitioner (NP) or Physician Assistant (PA) provides direct patient care in an inpatient hospital setting. Working collaboratively with the hospitalist physicians, the NP / PA will manage and coordinate patient care for hospitalized patients, assist with admissions, perform daily rounds, and provide medical interventions as appropriate.

Essential Job Functions

• Perform comprehensive assessments of patients upon admission, throughout their hospitalization, and upon discharge.
• Develop and implement patient care plans, ensuring evidence-based practices and protocols are followed.
• Diagnose and manage acute and chronic conditions and monitor patient progress.
• Prescribe medications and order diagnostic tests as needed, in accordance with hospital policies and state regulations.
• Provide education to patients and families regarding their condition, treatment options, and post-discharge care.
• Work collaboratively with the tele-hospitalist team, specialists, nurses, and other healthcare professionals to ensure comprehensive and coordinated care.
• Provide timely and accurate documentation in the electronic medical record (EMR) system.
• Participate in interdisciplinary team meetings and contribute to care planning and decision-making.
• Participate in quality improvement initiatives to enhance patient care, improve clinical outcomes, and increase patient satisfaction.
