This role involves collaboration with interdisciplinary teams, patients, and families to facilitate efficient transitions of care and promote optimal recovery. Key Responsibilities : Assess, plan, implement, and evaluate individualized care plans for assigned patients. Coordinate patient discharge planning and post-acute care arrangements.
