As a rapidly expanding primary care provider, we are looking for exceptional talent to join our dynamic team.
Key Responsibilities :
- Provide comprehensive care for patients with acute and chronic conditions, focusing on older adults.
- Take full accountability for patient outcomes while collaborating with specialists as necessary.
- Engage actively with hospitalists to ensure continuity of care for hospitalized patients.
- Responsible for patient assessment, diagnosis, treatment, care coordination, and documentation.
- Lead and mentor a dedicated care team, including medical assistants and care coordinators.
- Work with the transitional care team for patients unable to access office visits, ensuring they receive appropriate care in various settings.
- Facilitate weekly Super Huddles and Transforming Care Meetings to enhance team coordination and performance.
- Play a vital role in center management, including covering for other providers and actively recruiting new patients and team members.
- Mentor and coach primary care providers to achieve outstanding clinical outcomes.
- Partner with operational leaders to drive financial performance and increase patient membership.
- Carry out additional tasks as assigned to support organizational goals.
Qualifications and Skills :
Proficient in Microsoft Office Suite and other relevant applications.Willingness to travel locally and occasionally for training and recruitment purposes.Fluent in English, with other language skills being a plus.Demonstrated ability to exercise independent judgment in clinical care.Education and Experience Requirements :
MD or DO in a relevant specialty such as Internal Medicine, Family Medicine, or Geriatrics.Active medical licensure in the state of employment.At least 1 year of clinical experience preferred, with service as a Lead PCP at our organization being ideal.Completion of specialized training programs as part of professional development.Board certification in a relevant specialty preferred; board eligibility is required.Maintain board certification through continuing education and necessary assessments.Current DEA number for controlled substances is required.Basic Life Support (BLS) certification within the first 90 days of employment is required.Join us at our organization, where we are transforming healthcare for seniors, improving their well-being, and making a positive impact on our communities.
