Description and Responsibilities
Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model.
Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments.
Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals.
The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served.
The Program Coordinator provides oversight of the day-to-day operations including program staff, treatment, milieu management and administrative functions.
They also ensure the program adheres to contractual requirements and provides high quality rehabilitation according to each person’s Treatment Plan.
Other Key Responsibilities :
- Ensure optimum program coverage through the development and monitoring of the program schedule.
- Develop and monitor the implementation of all procedures that ensure the health, safety, and well-being of individuals served.
- Foster relationships with neighbors, local educational institutions, and the community.
- Participate in on-call rotation.
Qualifications
- High School Diploma, GED or equivalent, required.
- Experience working with adults with mental health challenges, required.
- Previous supervisory experience, required.
- Valid drivers license and acceptable driving record, required.
