About The Position

Experience : Minimum 1 year of Adult Home Health Experience or 1 year of Hospice Experience

License : Valid nursing license required

Certifications : CPR, BLS

Must-Have :

• Proficient in HomeCare HomeBase EMR and Oasis
• Recent home health or hospice experience
• Reliable transportation, current driver’s license, and agency-required liability insurance
• Willingness to travel up to 75 miles

Description :

Provides coordinates and directs the provision of home nursing care according to physicians' orders.

Nursing care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Degree of travel includes home visits and periodic office meetings.

Average number of patients per day is 5-8.

Visits may include Start of Care, Wound Care, and Follow-up Visits.

All documentation must be completed within 24 hours of the patient appointment.

Delayed and incomplete documentation impacting billing and follow-up patient care may lead to a cancelation.

Expectations include accepting every patient on the schedule, regardless of distance from previous appointments.

Clinicians may discuss any financial concerns with the appropriate team for possible solutions.

All work must align with productivity expectations and agency policies.

A commitment to providing quality patient care in the home health setting is essential.
