Responsibilities
Provide comprehensive patient assessments and develop individualized care plans.
Participate in continuous professional development and training programs.
Administer medications and monitor patient responses to treatments.
Set and achieve personal performance targets in patient care.
Educate patients and families on health management and disease prevention.
Conduct routine health screenings and preventive care services.
Collaborate with healthcare teams to ensure coordinated patient care.
Utilize evidence-based practices to enhance patient outcomes.
Manage patient follow-ups and ensure continuity of care.
Document patient interactions and maintain accurate medical records.
Engage in community outreach to promote health awareness.
Analyze patient data to identify trends and improve care strategies.
Experience / Qualifications
Valid Nurse Practitioner license and current CPR certification.
Strong commitment to continuous professional development and learning.
Proven experience in patient assessment and care plan development.
Proficiency in administering medications and monitoring responses.
Strong goal-setting skills and ability to achieve performance targets.
Ability to effectively educate patients on health management.
Strong skills in documenting patient interactions and maintaining records.
Bilingual Russian preferred
Must have 1 year of nursing exp
