About the Role

As a Nurse Practitioner with Matrix, you will conduct comprehensive health assessments during visits that last up to one hour. These assessments offer invaluable insight into members' overall health, which can be challenging to capture in standard office visits. Your responsibilities will include :

• Performing adult and geriatric assessments covering medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and documentation per Matrix protocols.
• Ordering and administering point of care and lab-based testing as needed.
• Collaborating with physicians, social workers, family members, and other healthcare professionals as appropriate.
• Working together with Primary Care Physicians (PCPs) on patient education and follow-up as needed.
• Providing services at the member’s residence, whether in homes or Skilled Nursing Facilities.
