Transformation House is licensed by the Minnesota Department of Human Services, we are a 245G Substance Use Disorder Treatment Facility. We have three residential sites, in Anoka and Fridley, and we have two sober living facilities, in Champlin and Fridley.

Description

We are looking for a passionate Dual Licensed LADC / ADC-T & / MHP / MHP Licensing Candidate ) (LICSW, LPCC, LMFT) to join our team. We use a multidisciplinary team model. You will work closely with our entire team to provide excellent care.

Job Responsibilities (not meant to be all-inclusive)

• Comprehensive Assessments
• Case Management
• Skilled in performing the 12 core functions
• Knowledge of Minnesota Matrix Six Dimension
• Required Documentation, completed on time per licensing requirements.
• Facilitate therapeutic group sessions
• Provide Individual Sessions
