Nurse Practitioner - Palliative Care

Findlay, OH

Seeking a compassionate and experienced Nurse Practitioner (NP) for a community-based nonprofit focused on palliative care, hospice, and geriatrics. This role involves delivering comprehensive, person-centered care, primarily (75%) inpatient, managing symptoms, advanced care planning, and hospice transitions. The NP will conduct rounds, manage outpatient / Telehealth visits, collaborate with specialists, provide independent care with backup, maintain documentation, educate staff, advocate for patient care, and handle up to 6 daily patient encounters, five days a week.

Requirements

Board Certified NP - REQUIRED

OH license (or willing to obtain) - REQUIRED

DEA (must be obtained prior to start) - REQUIRED

no exemptions

Experience as advanced provider in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) - REQUIRED

Clean malpractice and license - REQUIRED
