SWMCCC is dedicated to supporting individuals under community supervision helping them achieve successful reintegration and promoting public safety. We are looking for a compassionate and organized Case Manager to join our Community Supervision Program.

Job Description :

As a Case Manager you will work closely with clients in the community supervision program providing guidance support and resources to help them meet their goals and comply with program requirements. You will serve as a key point of contact for clients ensuring they have access to the services they need to succeed.

Responsibilities :

• Client Support :

Assist clients in identifying needs and accessing community resources such as housing employment medical care transportation and social services.

Answer phones schedule appointments manage calendars and maintain organized records.

Assist the Clinical Director with insurance billing tasks including verifying coverage gathering required documentation and preparing billing submissions.

Support internal communication between staff providers and partnering agencies.
