ELIGIBLE SITE FOR NHSC RURAL COMMUNITY LOAN REPAYMENT PROGRAM

Location : Family Care Center of Taylorsville, 1668 NC-16, Taylorsville, NC 28681

Licensed Clinical Social Worker provides directly or assists program staff in completion of the health and behavior screening, counseling and referral. If indicated, provides further psycho-social assessment, including diagnosis, treatment plan, and individual therapy to patients in outpatient setting.

Provides education or assists staff in developing education sessions on stress and anger management. Determines tools to be used for screening / assessing anxiety and depression, determines criteria to be used to assist staff in referring patients for further health and behavior services.

Psychosocial assessments and diagnostic evaluations with referred patients to develop a treatment plan. Provides individual counseling and / or educational services to patients referred. Coordinates and communicates activities related to patient plan of care / services rendered with the interdisciplinary team.

Maintains clinical and professional competency according to licensure requirements for Licensed Clinical Social Workers in the state of North Carolina and provides care appropriate for the age, health, and developmental status of patients served. Has full access to patient health information.
