Provides direct and indirect clinical social work and discharge planning services for patients and their families in various inpatient and outpatient settings. Participates as a professional team member in patient care planning along with other hospital and community agency personnel.
Frequent contact with patient care services staff, physicians, and ancillaries to discuss patient care and diagnostic needs. Participate in care rounds and other interdisciplinary collaboration within the in-patient and outpatient programs. Frequent contact with community agencies, third party payers, outside review organizations and case managers.
Essential Duties & Functions:
Conducts psychosocial assessment to determine transition planning and coordinates transition to community. Reevaluates periodically and revise plan accordingly.
Provides counseling and psychosocial interventions, is responsible for assisting patients and their families in understanding and learning to cope with illness and disability. Assists patients and families in resolving complex personal and environment issues, which interfere with affective social functioning and obtaining maximum benefit from medical care.
Provides education, consultation and acts as a liaison between the hospital, patients and families, community agencies and the multi-disciplinary team.
Provides assessment and referral in cases of abuse, neglect or exploitation and follows Ohio mandated reporting laws.
Provide thorough, accurate and timely documentation.
Education Required: Master's degree
Additional requirements:
Area of study or major: Social Work
Licensure/Certification/Registration:
LSW required; LISW preferred
Licensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist Board
Experience Required: 1 - 3 years of job-related experience
Preferred experience: Previous hospital or community social work experience required. Knowledge of community agencies preferred
Knowledge/Skills:
