FUNCTION : The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES : To perform this job successfully, an individual must be able to perform each key function satisfactorily.
Responsibilities : - Provides direct patient care, evaluates needs, consults with specialists, and adjusts care plans. - Designs and implements patient care based on assessments and involves significant others. - Conducts therapy within defined standards and directs support staff. - Communicates effectively to achieve positive patient outcomes.
