Job Summary :
As a Self-Direct Program Coordinator at Express Employment Professionals, you will be the key point of contact for members and caregivers. Your role involves managing and coordinating services, conducting home visits, ensuring compliance with program guidelines, and processing paperwork. You will work closely with members, caregivers, and internal teams to deliver high-quality support and maintain program standards.

About the Self-Direct Program :
The Self-Directed Program through Montana Medicaid empowers individuals with disabilities or chronic illnesses to take control of their own care by allowing them to choose their caregivers and develop personalized care plans outside of an institutional setting. Participants work with coordinators to tailor their services to meet specific needs, fostering independence and enhancing their quality of life. Eligibility typically requires meeting specific Medicaid criteria related to income and care needs.
Duties / Responsibilities :
Program Compliance :
Act as the Plan Facilitator, you are required to complete program specific documentation to maintain member authorizations.
Complete necessary trainings to maintain Plan Facilitator status at an agency and state level.
Travel
Applicants are expected to be comfortable with travel to fulfill in person visits every six months and year in accordance with state requirements. Our agency offers Self-Directed Services to the following counties : Flathead, Sanders, Lake, Mineral, Missoula, Ravalli, Granite, Powell, Lewis and Clark, Cascade, Deer Lodge, Silver Bow, Glacier, Pondera, Toole, and Teton.Intake & Paperwork :
Perform intake processes, including in-home visits, and complete all necessary paperwork.Update and manage documentation according to program requirements, including temporary authorizations for condition changes.Process all new caregiver onboarding paperwork to ensure labor / healthcare compliance standards.Coordination & Communication :Schedule and lead conversations with case management depending on individual members care needs.Handle health and safety issues and risk negotiations as per program parameters.Provide clear written and verbal communication to all internal and external staff in relation to member changes in condition, requirements of care, and documentation.Quality Assurance :
Participate in Quality Assurance Review (QAR) processes alongside the Healthcare General Manager to maintain agency standing.Required Skills / Abilities :

Education : High school diploma or college degree preferred.Administrative Experience : 5 years of administrative experience preferred.Home Healthcare Experience : At least 2 years of related Home Healthcare experience preferred.Aging and Disability Services : Minimum of 1 years experience in aging and disability services preferred.Medicaid Experience : Prior experience with Medicaid preferred.Physical Requirements :
Sitting : Ability to sit for extended periods while working at a desk.Lifting : Occasionally required to lift and carry items up to 15 pounds.Computer Use : Capacity to focus on and work with a computer screen for prolonged periods.Drivers License : Must have a valid drivers license and a clean driving record
