Job Description Summary
Liaison between Home Care and physicians, case managers, skilled nursing facility administrators and other referral sources in the community.
Provide education to referral sources and the public concerning Home Care's scope of services, and the appropriate Medicare and Medicaid benefit for these services.
Promotion of Home Care services to physicians, other referral sources, and consumers. Attend health fairs, community events, educational programs and other events to promote MUSC Home Health / Hospice services.
Assist in the admissions process by providing a presentation of services and assist the family / caregivers in the execution of admission paperwork.
Liaison between the KH Home Care billing team and the physician offices.
