Position Overview
Join a community-driven pediatric care team as an LPN Care Manager, where you'll coordinate and oversee holistic care for children and families. In this role, you'll serve as a vital link between providers, patients, and community resources-delivering clinical support, care coordination, and health education in a nurturing outpatient setting.
Key Responsibilities
Conduct intake assessments, care planning, and follow-up under provider guidance
Coordinate referrals, specialty appointments, and link families with community services
Monitor care plans, follow outcomes, and re-assess as needed to support effective treatment
Communicate with families to reinforce care instructions, manage chronic conditions, and answer questions
Collaborate with interdisciplinary team members to streamline care delivery and optimize patient outcomes
Document all care coordination and patient interactions in compliance with clinical protocols
Advocate for pediatric patients and ensure equitable access to quality services

Job Requirements
Current LPN licensure for the state of Vermont
Experience in pediatrics, care coordination, or outpatient care preferred
Strong communication skills and an empathetic, patient first mindset
Ability to collaborate effectively within a multidisciplinary healthcare team
