Education Requirements \nA.Completion of educational program for licensure as a LPN.\nB.Successful completion of required New York State Department of\nHealth LPN IV Therapy Initial Program for the demonstration of\ncompetence in the performance of intravenous therapy / blood\nadministration procedures as part of Centralized Orientation.\nC.
