American Traveler is seeking an experienced RN Clinical Care Coordinator with CA license and acute care coordination experience for a Case Management / Utilization Review role.

\n \n Responsibilities\n

  • Work in an acute care hospital setting on the Care Coordination (Case Management) unit
  • Focus on case management and utilization review, covering all adult areas of the hospital except women's health if not experienced
  • Typical patient assignment ratio is 1 : 20
  • Uses Cerner EMR along with Midas, InterCall, and Curaspan systems
  • 8-hour day shifts with every other weekend required; must be first to float if needed
  • Option for 12-hour night shifts is available only for candidates with ED background
  • Four-day virtual charting orientation followed by two days preceptor-based orientation
  • Responsible for care coordination, discharge planning, and utilization review for adult patients
  • Must be adaptable, a fast learner, and able to function independently with minimal orientation
  • Expected to maintain excellent attendance and follow break and lunch protocols
  • Must adhere to business casual dress code, including no open-toed shoes and minimal fragrance
  • Testing on basic or advanced dysrhythmia, and sterile technique required during orientation
  • Travelers are scheduled to work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year's), with holiday time off considered but not guaranteed
  • Candidates may not have been employed by CommonSpirit (including CHI or Dignity) in any capacity within the past year, except PRN through outside agencies
  • First time travelers are not accepted for this assignment

  • Active CA RN license required at the time of application; pending licenses not accepted
  • Current BLS certification required
  • Minimum 3 years of hospital experience, including acute care coordination
  • Proficient with Cerner, Midas, InterCall, Curaspan, and Microsoft Office programs
  • Experience in discharge planning and utilization review required
  • Must have prior acute care experience in a Dignity / CommonSpirit facility in CA
  • Must provide two professional references : one supervisor from the last 12 months and one peer or supervisor from the last 3 years
