American Traveler is seeking an experienced RN Clinical Care Coordinator with CA license and acute care coordination experience for a Case Management / Utilization Review role.
\n \n Responsibilities\n
Work in an acute care hospital setting on the Care Coordination (Case Management) unitFocus on case management and utilization review, covering all adult areas of the hospital except women's health if not experiencedTypical patient assignment ratio is 1 : 20Uses Cerner EMR along with Midas, InterCall, and Curaspan systems8-hour day shifts with every other weekend required; must be first to float if neededOption for 12-hour night shifts is available only for candidates with ED backgroundFour-day virtual charting orientation followed by two days preceptor-based orientationResponsible for care coordination, discharge planning, and utilization review for adult patientsMust be adaptable, a fast learner, and able to function independently with minimal orientationExpected to maintain excellent attendance and follow break and lunch protocolsMust adhere to business casual dress code, including no open-toed shoes and minimal fragranceTesting on basic or advanced dysrhythmia, and sterile technique required during orientationTravelers are scheduled to work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year's), with holiday time off considered but not guaranteedCandidates may not have been employed by CommonSpirit (including CHI or Dignity) in any capacity within the past year, except PRN through outside agenciesFirst time travelers are not accepted for this assignment\n \n Requirements\n
Active CA RN license required at the time of application; pending licenses not acceptedCurrent BLS certification requiredMinimum 3 years of hospital experience, including acute care coordinationProficient with Cerner, Midas, InterCall, Curaspan, and Microsoft Office programsExperience in discharge planning and utilization review requiredMust have prior acute care experience in a Dignity / CommonSpirit facility in CAMust provide two professional references : one supervisor from the last 12 months and one peer or supervisor from the last 3 years\n Benefits\n
