REPORTS TO : Director of Rehab
POSITION SUMMARY :
The Speech Language Pathologist will primarily provide outpaent rehabilitaon services in an in-home seng for the older adult populaon. The in-home seng could be a paent's private residence or in a paent's room in an assisted living facility, or in an independent living facility.

The Speech Language Pathologist organizes and provides speech-language services for the agency to facilitate rehabilitaon.
They direct paent parcipaon in selected tasks to restore, reinforce, and enhance performance. Also, the Speech Language Pathologist facilitates the learning of those skills and funcons essenal for communicaon and swallowing, to diminish or remediate disorders or deficits.
They direct acvies to promote and maintain health, within the scope of pracce and consistent with the Code of Ethics of the profession. This posion provides a flexible schedule and work environment in various sengs.
RESPONSIBILITIES / ACCOUNTABILITIES :
1. As necessary or applicable, supervision of CFYs temporary licensees, students and support personnel in accordance with state licensure requirements and professional standards.
2. Responsible for adherence to department and paent treatment schedule, mely clinical and billing documentaon requirements as well as work related meengs.
3. Aends and contributes to paent care, staffing conferences and other related meengs.
4. Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the paent.
5. Consults with other members of the rehabilitaon team and the physician to select the most appropriate therapeuc regimen consistent with the needs and capabilies of each paent.
6. Adapts program and / or plan of care according to the needs of the individual paent.
7. Promotes a safe environment and enforces the eliminaon of fire and safety hazards.
8. Orders supplies and equipment as necessary.
9. Cleans, maintains, idenfies, and reports the need for repair of equipment, as necessary.
10. Puts Paent Service first, ensuring that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individual's needs and rights.
11. Screens, examines and evaluates paents, including history, systems review, and applicaon of appropriate tests and measures, synthesis informaon and interprets examinaon findings in order to establish a diagnosis, idenfy impairments, determine the predicted level of improvement and the me required to achieve it, idenfy precauons / contraindicaons and design and plan of care.
Develops appropriate treatment goals and methods in collaboraon with the paent and caregivers, implements the speech language pathology treatment plan, and completes all related documentaon and record keeping regarding these services. Services include but are not limited to applicaon of therapeuc techniques in areas such as :
12. Coordinates the acquision of adapve communicaon devices and instructs paents in their use.
13. Facilitates idenficaon of hearing deficits and appropriately refers for assessment, diagnosis rehabilitaon and / or personal amplificaon.
14. Selects appropriate acvies suited to the individual's physical and mental capacity, intelligence level and interest.
15. Instructs paents in the skills and techniques required for parcipaon in therapeuc acvies, and evaluates paent progress, atudes and behavior as related to rehabilitave potenal.
16. Performs roune reexaminaon as needed / required to modify / progress plan of treatment.
17. Coordinates care in the home and community with other members of the interdisciplinary team in collaboraon with the referring physician.
18. Performs other related dues as required.
SPECIFIC EDUCATIONAL / VOCATIONAL REQUIREMENTS :
1. Must have a valid Speech Language Pathologist license in the state(s) of pracce, or proof of license eligibility.
2. Must possess or be eligible for a Cerficate of Clinical Competence from the American Speech-Language-Hearing Associaon (ASHA) or be eligible to parcipate as an ASHA Clinical Fellow.
3. Must have a valid driver's license or reliable, independent transportaon to and from the paents' residence.
