Job Description :

✅ Provide skilled OT services to adult and geriatric patients in their homes

✅ Perform evaluations, develop individualized care plans, and deliver therapeutic interventions

✅ Accurately document services using EMR system : Wellsky (Kinnser)

✅ Educate patients and caregivers to improve independence, safety, and quality of life

✅ Collaborate with nurses and therapists for patient-centered outcomes

What You Need :

✅ Active Occupational Therapy license in the state of Texas

✅ Graduate of an accredited OT program

✅ CPR Certification

✅ Reliable transportation & valid driver’s license

✅ Auto liability insurance

✅ Experience with geriatric and / or pediatric population preferred

✅ Strong communication and time management skills

✅ 1+ year of OT experience preferred

✅ A passion for patient-centered care and home health
