Job Summary :

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and managing cases to ensure optimal outcomes. Responsibilities :

• Conduct comprehensive assessments of patients' health status and develop individualized care plans
• Coordinate and manage patient care throughout the healthcare continuum
