The Medical Assistant Position will be working with the Nurse, and Case Manager to ensure that appropriate medical care is provided to individuals receiving services from Strive. The Medical Assistant will work closely with each Individuals Team and serves as a Medical Liaison with other departments and with outside medical professionals as needed. Medical assistant will provide training to Direct Support Professionals.

Requirements :

• Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
• Coordinates and monitors annual reviews
• Will complete monthly monitoring reviews
• Will run monthly blood sugar reports and follow up with any missing information.
• Reviews weights monthly, follows up with any missing documentation, report to nursing if any weight changes per SOP.
• Recording and updating patient medical histories.
• Measuring and recording individual vital signs.
• Completing delegated tasks including but not limited to; G and / J tube feedings / medication administration, wound care, catheter care.
• Will review GER’s (General Event Reports) daily and get any follow up needed by the nurse.
• Review t-logs daily and get any follow up needed by the nurse.
• Transcribing information from medical professionals into electronic health records including but not limited to; medication orders, care plans, and updating appointments.
• Will take part in educating Direct Support Professionals, contracted providers, and caregivers as needed with; infection control, health record documentation, and follow up or anything else health related.
• Monitor and track care plans needing updated per changes or annually.
• Will review and update care plans for nursing review.
• Faxing doctors for the nurse and completing follow up as assigned.
• Prepare, develop, and revise forms and reports as needed.
• Answering the nursing phone, directing, and taking messages accordingly.
• Maintain excel spreadsheets used for monitoring and tracking.
• Communicating with health care providers (physicians, clinics, therapists, etc.)
• Filing, Scanning, and attaching documents.
• Maintain nursing calendars.
• Assisting with the maintenance of equipment and ordering supplies for nursing department. Maintains nursing storage room and setting time with nurse for drug disposals as needed.
• Other duties as assigned by the nurse.
