Details :

Setting : Provide Hospice / Palliative Care for Patients

Work autonomously in an APP role

Work collaboratively with the Palliative Care Medical Team to deliver appropriate care from initial assessment through discharge

Assessments include : disease-specific changes, decline or change in function, nutrition and / or cognition, inadequately-controlled pain or other distressing symptoms, falls, infections, impact of disease burden on patient, caregiver, and / or family's quality of life

Prescribe medications, order / interpret labs, order treatments or medical equipment

Have meetings with Palliative Medical Director as needed, but no less than biweekly

Mileage reimbursement provided

4x10s - no weekend schedule

Required :

Minimum of 2 years of NP experience required

ACHPN certification preferred

Active BLS and DEA required

Compensation :

Receive a paycheck every Friday
