POSITION SUMMARY:
The Medical Case Manager will be responsible for ensuring patients receive the necessary medical, psychological, and social support required for optimal health outcomes, including Ryan White requirements. This position requires excellent care coordination, strong patient advocacy skills, and a deep understanding of HIV including HIV case management.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Conduct comprehensive assessments of HIV-positive patients to identify their medical, emotional, and social needs, including Ryan White requirements.
- Develop individualized care plans that address the patient's specific needs and ensure continuity of care.
- Coordinate care with healthcare providers, specialists, social services, and community resources to optimize patient outcomes.
- Provide education and counseling to patients about HIV, treatment options, medication adherence, and health maintenance.
- Advocate for patients to ensure access to necessary medical services, support programs, and resources.
- Monitor patients' progress through regular follow-up visits and make adjustments to care plans as required.
- Maintain accurate and up-to-date patient records using EPIC and CHAMP Systems.
- Collaborate with the multidisciplinary care team to ensure holistic, patient-centered care.
- Address any barriers to care, such as transportation, housing, or financial challenges.
- Participate in ongoing training and professional development opportunities.
QUALIFICATIONS:
- Experience: A minimum of two (2) years of experience working with HIV-positive patients AS A Case manager or medical case manager.
- Education: Bachelor's degree in Social Work, Public Health, or a related field is preferred
- Language Skills: Bilingual proficiency in Spanish is preferred to serve a diverse patient population.
- Technical Skills: Experience using electronic health record (EHR) systems, particularly EPIC and CHAMP Systems, is preferred.
- Certification: Case management certification (e.g., CCM, CMC) is preferred
- Soft Skills: Strong communication, organizational, and interpersonal skills. Ability to work effectively in a team environment and independently.
- Knowledge: Knowledge of HIV care, including HIV treatment protocols, and HIV community resources is essential.
