This position involves providing in-home nursing care to patients, ensuring high-quality, family-centered services. As a key member of our interdisciplinary care team, you will conduct patient assessments, develop individualized care plans, and collaborate closely with patients, families, and physicians to promote positive health outcomes.\n\nEssential Functions : \n
• Perform initial and ongoing patient assessments in the home setting.\n
• Complete thorough and accurate OASIS documentation.\n
• Develop and implement individualized care plans based on patient and family needs.\n
• Deliver professional, safe, and effective nursing care according to established standards.\n
• Educate patients and caregivers throughout the care process and during discharge planning.\n
• Coordinate with community resources and other healthcare professionals to optimize care.\n
• Supervise and provide guidance to Licensed Practical Nurses (LPNs) and Home Health Aides.\n
• Document patient progress and revise care plans as needed to achieve desired outcomes.\n\nQualifications : \n
• Current Registered Nurse (RN) licensure in the State of Maryland.\n
• American Heart Association BLS CPR certification (for healthcare providers).\n
• Valid driver's license in good standing and reliable transportation.\n
• Graduate of an accredited School of Nursing.\n
• Minimum of 1 year of medical-surgical (med-surg) experience preferred.\n
• Strong clinical assessment and critical thinking skills.\n
