About The Position

Description :

Seeking a Care Manager who is experienced in hospital care management and discharge planning.

The position requires floating between units as needed.

Weekends are required, with two weekend shifts each pay period.

Travelers may request standard and / or holiday time off, but approval is at the client's discretion.

All required charting will be done using a specific electronic medical record system.

Candidates will need to document patient care, including assessments and discharge planning.

The successful applicant will participate in care coordination activities.

Experience with various patient populations, including those with psychiatric disorders, is preferred.

Knowledge of advance directives, guardianship, and home healthcare is essential.

The ability to coordinate services for patients after discharge is a critical component of this role.

Candidates should possess strong communication skills and the ability to work as part of an interdisciplinary team.

All documentation must reflect compliance with relevant care standards and regulations.

This position also involves educating patients and caregivers to facilitate successful discharges.
