Physical Therapist – Gwinnett County Jail (Lawrenceville, GA)

TransPerfect Health is seeking a Physical Therapist to provide care every Tuesday and Thursday at the Gwinnett County Jail in Lawrenceville, GA. This is a co-ed facility , and all therapy sessions are conducted within the infirmary.

Compensation : $85.00 / hour

EMR Experience : Net Health experience is a plus.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans with measurable goals and defined time frames
• Prepare and submit clinical documentation and progress summaries based on identified and achieved goals
• Collaborate with other healthcare professionals to ensure they have the information needed to appropriately evaluate and treat patients
