Nurse Practitioner Telehealth

A company is looking for a Nurse Practitioner to join their Connected Transitional Care team.

Key Responsibilities

Conduct remote follow-up visits for patients discharged from the hospital using a telehealth platform

Collaborate with primary care team members to enhance patient care and access

Promote a team-based approach to patient management within the healthcare system

Required Qualifications, Training, and Education

Inpatient, ER, or primary care experience as a Nurse Practitioner

Active Nurse Practitioner license

Experience with telemedicine services preferred

Ability to work collaboratively in a team environment

Commitment to improving patient outcomes and access to care
