We?re seeking a provider who is bilingual in English and Spanish with a strong commitment to preventive care and evidence-based medicine.

• Base salary: $120,000 ? $130,000, with 10%?15% performance-based bonuses

Responsibilities

• Average 10 to 15 patients per day
  

• Focus on chronic disease management
  

• Develop strategies to reduce ER visits and hospitalizations
  

• Collaborate with Case Managers and Specialists
  

• Educate patients on preventive care and treatment adherence
  

Requirements

• Experience in value-based care ? Required
  

• Licensed PA in the state of Florida ? Required
  

• Fluent in English and Spanish ? Required
  

• Experience with eClinicalWorks (EMR) ? Preferred
  

• Passion for caring for a senior population
