Nurse Practitioner / Physician Assistant - Inpatient Palliative Care Consults

Work Schedule :

100% FTE, day shift working Monday - Friday, 8a - 5p. You will work at University Hospital in Madison, WI.

As an Advanced Practice Provider, you'll play an integral role on our team by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner or Physician Assistant to :

• Provide complex symptom management recommendations.
• Work in collaboration with the Palliative Care Medical Director, attending medical team, and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases.
• Effectively communicate with patients and family by utilizing concepts of motivational interviewing.
• Perform assessment process to include patient history, physical, psychosocial, and spiritual assessment.
• Provide support, counseling, and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness.

At UW Health, you will have :

Education : Minimum -

If Nurse Practitioner :

Master's or Doctorate's degree from accredited Nursing Program.

If Physician Assistant :

Bachelor's or Master's degree from accredited PA Program.

Licenses and Certifications :

Minimum -

If Nurse Practitioner : Active RN and APNP license in State of Wisconsin AND active board certification as a NP in Adult; Adult / Gerontology Acute Care; Adult / Gerontology Primary Care.

If Physician Assistant : Active PA license in State of Wisconsin AND active PA board certification.

All Nurse Practitioner and Physician Assistant applicants must have : Active DEA license or ability to apply for such license prior to or upon hire AND CPR certification and advanced life support certifications if relevant to practice area (ACLS, PALS, NRP).
