Your Mission : Rock Creek Foundation is seeking a highly motivated, independent and relationship-driven Full-Time Bilingual Case Manager to join our team. The Case Manager will be responsible for assessing, planning, implementing, monitoring, and evaluating the actions necessary to meet each individual’s health and human needs.

What You’ll Do :
Assesses clients' social, emotional, financial, health, vocational, or other problems to develop rehabilitation treatment plan, or Individualized Plan goals for Person-Centered PlanResponsible for scheduling, attending, writing, and implementing Person-Centered PlanComplete Intake / Assessment process for all new DDA Program, Employment Services, CDS, PSS and Residential individuals in both English and SpanishEnsures that all Person-Centered Plan goals are inputted in Electronic Medical Records and monitors tracking on all goalsMonitor monthly goals, activities and track progress, ensures that goals are CARF compliantLiaison between individuals, families, and any other community service providerDocument case records and other agency required documentsAssists with reporting, documenting and provides appropriate response time for all internal / external incidents per DDA / MHA Policies on Reportable IncidentsIdentify and coordinate behavioral services when needed, makes referrals as neededPrepares and maintains written case records, reports, performs case follow-up and closing, and performs other administrative tasks as requiredAssists in conducting all quarterly chart auditsConduct weekly and / or bi-weekly service notes in iCM to ensure compliance with QIO / CQL standardsCoordinate and implement general departmental projectsResponsible for ensuring staffing schedules and ratios are in compliance per individual’s plan / treatment planRespond to and resolve customer complaintsAttends all mandatory meetingsOversees the Electronic Medical RecordIs on-call for all departmental needsParticipates in and successfully completes all required trainingsProvides training to all new employees and / or contract staff on iCM shift notes, attendance, etc.Oversees Quality Assurance measures at all assigned licensed sitesAdheres to all County, State DDA, MHA, and MBON policies and proceduresOther duties as assignedWhat We Require :

High School Diploma or GED, or Bachelor's Degree in a human service or Related field of study; or Associate's Degree and (1) year of experience providing support to adults with ID / DD and SPMI.Two (2) of case management and / or coordination of care; or equivalent combination of education and experience.Must possess a valid driver’s license with less than three (3) points as well as current automobile insurance.Knowledge of internet and Microsoft Office software.Ability to pass background checks and FBI fingerprinting.What We Offer :
