Job Description

Job Title : Case Manager (RN)

Location : Morristown, NJ – 07960

Duration : 13+ weeks (Possible extension based on performance)

Schedule : 8 : 00 AM – 4 : 00 PM (May require weekend coverage)

Position Summary

The Case Manager is responsible for coordinating patient care throughout the acute care hospitalization, ensuring safe and efficient transitions of care, and supporting high-quality clinical outcomes. This role requires strong clinical judgment, effective communication, and expertise in discharge planning and utilization management.

Required Skills & Experience

• Minimum of 2 years of acute care experience strongly preferred.
• Previous experience in Case Management, Managed Care , and In-Patient Case Management is required.
• Strong communication, critical thinking, and care coordination skills.
• Experience in discharge planning is required.
• NJ candidates preferred; local applicants strongly considered.
